Sometimes the problems are not on the email server itself. The bottom line is on the MX entry of your mail server into your DNS Server or if you are using local/Private DNS Server try to switch first your dns server program to djbdns. This dns program is 100% compatible with qmail program, then start editing/optimizing the /var/qmail/supervise/qmail-send/run scripts.
