you may want to try a proxy fliter like ASSP (http://assp.sourceforge.net). mail proxy filters
are great since you don't have to mess around much with whatever MTA you're running.

if what you mean is blocking attachments from your users to anywhere, you must make them pass through ASSP first before going through your qmail. in my case, what i did was to run ASSP on the standard SMTP port, that is 25, then configured my MTA to run on a different port. But if you installed your ASSP in a diff. box, there'll be no need of doing it. You just point your users to your ASSP instead of directly pointing them to your MTA.
