I have seen video conference between Linux and Windows using the former
gnome meeting and netmeeting using the H.323 protocol, but it wasn't the
open source version. I imagine that that works too.

PS I can find out more details on the H.323 stuff if needed. I have a
good reliable source. proof: He writes the stack himself :)
