I use Xmission with Utopia for my business needs.  It's 15 MB
download/upload (full duplex), although I personally have only seen it at 12
MB, with something like 30 or 40 GB monthly traffic.  That's just the
personal line.  The business line has more throughput (30 MB) and more
traffic, something like 100 GB/month.  They support static IP addresses
(I've got 8 right now - 5 usable).  I currently just have two linksys
routers that are daisy chained for my internet -> external -> internal
network.  For the price, I can do port forwarding, block ports, and setup a
dmz.  I suppose that with a more robust router (i.e. cisco or a linux box
with dhcp, iptables, routes and nat) I could handle several more systems
without too much of a hassle.

I've never waited on hold for more than a few minutes and they also operate
an IRC channel that's available 24/7 including holidays.  The techs are
extremely knowledgeable and very helpful.
