I have been having lots of success with the recent waves of spam with a SA plugin called "Botnet". It uses DNS and keywords in hostnames and has really helped.
http://people.ucsc.edu/~jrudd/spamassassin/*Botnet*.tar<http://people.ucsc.edu/%7Ejrudd/spamassassin/Botnet.tar> I also setup FuzzyOCR <http://fuzzyocr.own-hero.net/>, which only gets called on mail that comes in under the spam threshold, and it catches most of the image spam right now.
