I tend to use LastPass for websites, and KeePassX for everything else. I seem to recall having opened the KeePassX database on a Windows computer at least once; however, I haven't yet tried it on Mac, and don't yet have a handheld with a useful browser. My biggest gripe with them is the ease with which they highlight security problems with websites. In particular, certain websites, including ones with extremely sensitive data, silently truncate new passwords.
