I have a carrier account with icall I think you can still get unlimited inbound for $7/month, otherwise inbound is $0.01/min. Outbound varies by destination (usually pennies/min or below even). If you don't want a full PBX for features, you can just use any SIP softphone or get a small linksys ATA to power a couple of handsets. If you want a simple, nice SIP PBX I recommend sipxecs (which includes freeswitch btw). I built a small call center with it and couldn't be happier with it.
