I've used ArrivalTel in the past, and was pleased with them.  I switched 
to Flowroute a while back.  They don't bundle anything, you just pay for 
what you use.  Based on what I'm doing (I have a few DIDs), it is 
cheaper for me.  For my home and business combined, we're paying about 
$15/mo.
