I've done this--it can be quite involved.

Just having an app start as a service at boot is fairly trivial--you just
set the sysv/openrc/systemd config, but doing so on a bootable cd requires
tweaking whatever tool you use to make that change occur in the resulting
image. That's what I thought you meant.
