Your problem is distinctive in that it happens in standalone and I assume playing through the PC speakers not to a file.
This is very unusual but it has been reported a few times on mplayer forum a good while ago and before the Bandwidth option was implemented - but its cause wasn't identified. So far the info I have is that it doesn't seem to happen with the "as shipped" setting except for links with long transmission times (e.g. satellite).
