The TRACK part of the name shouldn't be part of the format. If you still select CUSTOMSCAN_TRACK_CUSTOMTAG_PERFORMER in Custom Scan settings and use CUSTOMSCAN_CUSTOMTAG_PERFORMER instead in the slimserver title format settings it should work.
