I really hate to disagree -- as obviously you know more about the code than I do -- but this has just started on my system, and I have always used the latest dev builds. I can reproduce for you if you'd like. Older skin builds (I keep them around) show the 3-dot menu; the latest shows the green icon. I suppose it is some interaction with other aspects of our LMS setups that have given us different results (until now). I only pointed it out because it had just started for me and I thought you'd want to know. Certainly not worth worrying about.
