I've seen this error with MPEG4 files with leading mdat (i.e.index is after audio data which is not good for streaming) so files needs to be transcoded even for player than can play MPEG4 natively. A fix fwas committed to LMS last July which I thought fixed thios issue. Perhaps it is related. What sort of files are being played ?
