I have these issues with WASAPI also.  Running in shared mode, I get
what you describe when I had Default Format set to 48000Hz, setting
Default Format to 44100Hz made it work for me.  Alternatively, I could
leave Default Format at 48000Hz and use squeezelite parameter "-R -u E"

To use exclusive mode, I think you have to add some additional
parameters to the player, "-a :1".  But I could not get that to work on
my system.

squeezelite gives no indication of an error, so SLX does not know
anything is wrong.

My old ears can't tell the difference, so I always use "Primary Sound
Driver [Windows DirectSound]" and everything always works.
