Agreed, I've seen it happen with the Transporter, too. And yes, I've looked into this before, tried to reproduce in many ways. But I've never been able to so on purpose. It happens sometimes, but I don't know when.
Michael, please take a look at the dump; I'm sure it will give you some hints hopefully.
Unfortunately all your server.log shows is that it's missing the PLUGIN_RADIOTIME_MODULE_NAME string for whatever reason (did you disable the Radio plugin?). And that you're running a rather old LMS :-).
