*Crtl-Shift-D* to see debug options. There you will see a reset option (though uninstalling and reinstalling should do the same thing). You can also turn on file logging and enable more verbose debug logging. Two Defaults in output device list? That is weird. One of those is explicitly put there by SLX. Everything else in the list comes from calling a library to enumerate actual output devices. This sounds like a bug somehow, as everything from the libraray call would include [...] in it. There is nothing wrong with your squeezelite parameters, so we'll need more info to figure out what happened.
