I found the culprint. I have two similar actions "Add" and "Add all" - both should have the same shortcut, but one is wrong (does not use shift key). Which one appears in the toolbar depends on the listing. For exampe showing all albums (when there are more than 10) of search results will use "Add all" - and that has the error.
