Something I've mentioned previously on the Music Similarity thread - a scenario were you end up with an "extra track" queue - and thereafter, sometimes that becomes a misaligned track/cover-art image... I'm not sure if there's anything in this, but I thought the other day that ampersands in the playlists might be contributing to this problem, and I just saw the same thing again. i.e. track titles with "&" in them - like, "The Remains of Rock & Roll" for example.
Just a thought...
