Have you restarted SlimServer after the installation ? If you have and it still doesn't show up, could you restart SlimServer and then directly afterwards goto: http://localhost:9000/log.txt And see if you can see any errors in the log.
When installing you should unzip the zip file directly in the Plugins directory. When doing this you should get a TrackStat directory inside the Plugins directory and a Plugin.pm inside the TrackStat directory.
