Found the icon problem - the Lyrics icon is a JPG. If you convert it to a PNG (and update install.xml accordingly) it displays perfectly.
It looks like the server automatically resizes the icon images before displaying them (the displayed image is converted by the server from icon.png to icon_25x25_f.png).
