If both the SB3 and the amp are close to each other then what might be happening is that the amp is seeing the IR signal from the SB remote at the same time as the SB3 is sending the blaster signal. They can interfere with each other. When you press test only the blaster signal is being sent so no interference.
To test if this is it cover the infrared window on the amp with something solid (or make sure it isn't in view from the remote). If it works perfectly then the answer is to arrange for the infrared window to be covered. If you feel you can't cover the window because you may still want to use the amps own remote then enable the IR-Repeater function in the IR-Blaster config.
