It sounds like you have done the upgrade correctly, you can try to shutdown SqueezeCenter and then in the SqueezeCenter cache directory delete the file plugin-data.yaml and startup SqueezeCenter again. I'm not sure exactly where the SqueezeCenter cache directory is stored in a Mac. If that doesn't solve it, you can check to make sure there isn't an old Custom Scan version installed in some other plugin directory. I think SqueezeCenter will load plugins from "Plugins" directories in some various places.
