It sounds like you have done the upgrade correctly, you can try to
shutdown SqueezeCenter and then in the SqueezeCenter cache directory
delete the file plugin-data.yaml and startup SqueezeCenter again. I'm
not sure exactly where the SqueezeCenter cache directory is stored in a
Mac.

If that doesn't solve it, you can check to make sure there isn't an old
Custom Scan version installed in some other plugin directory. I think
SqueezeCenter will load plugins from "Plugins" directories in some
various places. I'm not sure exactly where these are placed on the Mac,
but I'm guessing that one is somewhere under the SqueezeCenter
installation and one might be somewhere below the user home directory.
