First of all the obvious - the checkbox next to the plugin name on the Plugins tab is ticked?
Otherwise I suggest you try going the new automated way, using Extension Downloader. Still slightly unintuitive to my mind, but does a good job of managing plugins. Go the the Advanced tab on the SC Settings page and then look in the drop down list for Extension Downloader. In the Additional Repositories box at the top cut and paste this url http://www.herger.net/slim-plugins/repo.xml and then click on Apply at the bottom of the page. You then need to restart SC to get it to see the new repo. When you next open the same Advanced|Extension Downloader page you should see then MIS plugin ready for selection. I think you then need to do a further restart to get it activate and then off you go. If your original MIS install was actually there you may see a message about needing to remove it first.
