Sounds like something is wrong with your installation. - Have you set/changed the directory parameters in the Custom Browse settings page ? What are they set to ? They should point to empty directories somewhere outside the plugin installation.
- Have you installed it through Extension Downloader or did you manually download and unzip the zip file ? If you unzip the zip file manually I suspect the problem is a bug in the Vista unzip tool, try using the Extension Downloader instead which is able to unzip the zip file correctly. You should get a fairly long list of menu types under the "Select type of menu to create" text.
