Would it be possible for you to enable "info" level logging on "plugin.customscan" in Squeezebox Server Settings/Advanced/Logging and post the log after you have performed a rescan ?
Can you also try to start a rescan manually from the "Custom Scan" settings page to see if the problem only happens during automatically triggered rescans.
