The log says:
Adding HTML directory: [C:\PROGRA~2\SQUEEZ~1\server\Plugins\PlaylistMan
6.0\PlaylistMan\HTML]
I think it's supposed to say:
C:\PROGRA~2\SQUEEZ~1\server\Plugins\PlaylistMan\HTML
And not have the intermediate "PlaylistMan 6.0" directory.
The zip looks correct when I download it manually so I think the issue
is how you performed the installation.
I suspect you unzipped it so it created its own "PlaylistMan 6.0"
directory and put all files inside that, which would explain why you
are seeing the issue with the manually installed version.
For the automatically installed version it should just work unless the
space in the zip file name cause some issue but I don't think this has
changed recently so I suspect the issue with the automatic installation
is something completely different.
