Had to sign up to post a comment to the thread in case it helps anyone
in similar situation.

I have been having issues with the Spotify plugin that had similar
symptoms to those described by people whose firewalls were blocking the
help app's port: Can browse Spotify and select tracks to play but there
is no sound coming out of my SB devices.

If I restarted the LMS manually it would then work fine so I knew my
firewally couldnt be the real source of the problem. However when my
server (windows 7 x64) rebooted I would be back to silence until I
manually bounced the LMS.

A comment by Triode back on page 410ish of this thread gave me a hint.
Maybe the order in which my Windows services were starting was causing
the issue. I changed the startup mode for the LMS to Auto (Delayed
Start) rather than just Auto and this seems to have cleared the problem.
There must have been some clash between Norton Internet Security and LMS
but by delaying the LMS startup the problem has gone away :)
