If it's too big for iPeng, iPeng will crash or show the note icon placeholder. That should only happen with _really_ large images these days, iPeng easily swallows 6,000x6,000 artwork which is why it no longer requests scaled artwork for the main screen.
I think this is an issue with the artwork cache on the server side, maybe it's too big or broken. You said you cleared the caches, did you clear the artwork cache, too? As ridiculous as it sounds, but 533 MB for the artwork cache file isn't really big, my artwork cache file has a similar size (575 MB) and has been much bigger in the past (over 2 GB). Another reason could be that LMS is no longer able to access the original image.
