just run "mapserv -v" for your compiled Mapserver CGI and if it does not contain "OUTPUT=PNG" then the support for PNG is missing. Or you have some broken dependencies, check also the apache error log for segmentation faults if you use MS 6.0. It has a re-written/re-structured PHP Mapscript code which might have some bugs left.
