I think there is a misunderstanding of what constitutes markup and what markup exressions. I just looked at http://www.pmwiki.org/wiki/PmWiki/CustomMarkup It describes how to create custom markup, but nothing about custom markup expressions as the core expression explained in http://www.pmwiki.org/wiki/PmWiki/MarkupExpressions Moreover http://www.pmwiki.org/wiki/PmWiki/CustomMarkup has a heading "Define a markup to call a custom function that returns content" which has as example the creation of markup for generating a random number. This is now usually done with the markup expression {(rand)}.
