Just an idea: if I understand correctly, the target attribute (target=
in a wiki page file) contains a list of pages the current page points
to.

Couldn't there be another attribute like DependsOn= that would list
all the pages that are required to display the current page (includes,
headers, etc.)

If this could be done, then the caching mechanism could check is any
of these files is more recent that the current cached version to
determine if it is outdated.

Would that help ?
