OK, I'm not sure if this is related, but I discovered
some weird repeatable behavior. When first accessing
a site with a query string (?A=Foo), pmwiki strips the
query string and redirects to the stripped page URL.
Once the SiteAdmin.Status page is created this no
longer happens. If I delete the Status page it does
it again! Perhaps this lead me to believe that it was
failing... on my install.
