Thanks for writing back Hans You're right, it is not there in FireFox 2 but the extra bar/line is there in IE7. I normally use FireFox but had been doing something in IE7 and continued the work there. Then I forgot to check FireFox.
You're also right about the top menu items. I was asked to turn them off and now they only show when I'm in the edit mode.
