Hi, James, welcome!

RecentChanges is already configured to display the author of the change, 
however, it might take some work to always get the author. What you're 
currently seeing is the case when there is no current author given (The ? is 
actually saying there is no profile page associated with the last person who 
edited that page.) 

When you edit a page, you'll see the Author field in the form under the page 
edit text box. Filling this in will also fill in the last field in the 
RecentChanges page. If you are allowing cookies, it will be filled in from then 
on until the cookie expires.

Requiring the user to enter their name in the Author field is discussed in 
http://www.pmwiki.org/wiki/Cookbook/RequireAuthor -- which you should read 
fully as well, as it brings in some other goodnesses you'd probably like.
