The patches have been added to the code and the problem has been fixed.  It
should work either way.

I've tested this under both the 5-9 build and the 5-11 build, and get the
same results. 

Compile the source, and run it passing in the name of the output file to
generate as a command line parameter.  Open the file with Excel, and go to
print preview, you should only see the right header and footer values.
