I discovered that the logs I provided were skewed. After a full day passed, I 
received 3 bounce backs, and realized those 3 connections that were relaying 
over port 10031 had been sent soon after the first setup and were stuck in the 
queue (arg!). So those are cleared out now. I think the new emails are never 
even getting placed into the deferred queue at all. Here is what the logs say 
after performing the test with throttle limit set to 1 and a queue flush:

After 10 minutes or so, there was still no activity, so that is why I know it 
isn't getting dropped into the deferred queue. Qshape confirms this as well.

I see that two related processes are running: /usr/lib/postfix/master and 
/usr/sbin/postfix-policyd -c /etc/postfix-policyd.conf. Is that what you mean 
by two instances? There aren't any other postfix and/or policyd processes 
running. Do you still want the results of postconf -n?

I could provide the contents of master.cf if you think that would help.
