How do you figure out that "http://"; is missing in the first case: polipo logs,
browser logs, tcpdump?

Since the problem goes away when you restart the browser, while leaving polipo
running, it's most certainly a bug in the browser.  And because we don't have
any detail about this piece of software or the system as a whole, it's hard to
understand what is going on.
