It may not be a big problem for the NTP server, but as the target of the
attacks it's a huge issue.

My server handles ~150qps, and only like 4-5 clients queries more than
once per minute, sometimes they do once per second also. Load on the
CPU by NTPD is 0.5%, and upload is well under 100kbits.
