The patch is required to correctly determine the homedir. I cannot exactly tell you why, anymore, but without it the homedir will be incorrect and statistical bayes data for future spam analysis can't be saved anymore. The same patch is used on FreeBSD.

Regarding the relay/listen defaults - I guess those are dependent on the use-case. Not sure what's appropriate to be shipped. In my (and other cases, e.g. FreeBSD port) the mail pipeline goes like this:

OpenSMTPD :25 -> spampd :10024 -> Dovecot LMTP delivery agent :10025
