is it possiblöe that you have a lot of relayed or via transport delivered mails at the same time? if it is so you should limit "smtp" and/or "lmtp" processes in the "master.cf", since cleanup has here in the "master.cf" a process-limit of 0 i think they would be triggered from many at the same time successful delivered messages
