You might want to take a look at the original milter documentation which comes with sendmail - it should hopefully answer (most of) your questions.
"order": as specfified in the configuration file. "reject": equivalent to an SMTP reply code 4xy or 5xy. "override": if "reject" is used by a milter, then "later" milters are not relevant anymore.
