The problem will live in the function that casts from Box* to GEOMETRY. PostgreSQL does automatic casting, so

Extent(GEOMETRY) return BOX*
Centroid(GEOMETRY) returns GEOMETRY

PostgreSQL will rewrite Centroid(Extent()) to Centroid(Box2Geom(Extent())
