Hi David, Which versions of PostGIS are you using? (SELECT postgis_full_version()). If it's earlier than 1.2.1, it sounds like you could be hitting the GiST index bug documented here: http://postgis.refractions.net/documentation/manual-svn/release_notes.html#id2692480.
