I have now changed gid to rid, and can see that gdal_translate creates an image:
gdal_translate -of GTiff -outsize 100% 100% "PG:host=localhost dbname=xxx user=xxx password=xxx schema=raster table=kommune_r" C:\spatialsuite\temp\kommune_r.tiff but shp2img -m C:\..\itasca.map -o C:\temp\shp2img.png -l kommune_r returns: msProcessProjection(): Projection library error.
