It seems to me that three things need to be blocked:
1) clicking the start menu
2) pressing the windows key (left or right)
3) pressing the CTRL-ESC key combination
To block #2 I I used Keytrap Config (ktconfig.exe) to add the
following two lines to my keytrap.ini
disablewindowskey=0,0,0,91,91,0,,,,0,1,0,0,1,0,,,,,,trap left windows key and
do nothing
disablerwindowskey=0,0,0,92,92,0,,,,0,1,0,0,1,0,,,,,,
