The buttons are not wide enough so the icons do not fit on them. Try setting width 20 for every button. Possibly just setting the first item in the list to width 20 will be sufficient or you might need to set them all. I notice the only item with the width specified in your exported command list is a BarVerticalLine (a separator) which you set to width 8.
