Whenever I've had this problem, usually it was because the cart didn't seat properly. When you insert the cart, sometimes you have to keep a little pressure on it w/ your fingertip as it seats (I think of it as giving a little extra 'boost'). Sometimes using a different drive will help. But sometimes the cart is just bad and that's that.
