Have you tried booting without extensions?

Have you tried both the upper and lower slots?

I remember this happening once with my 1400cs, BUT I CAN'T REMEMBER HOW I
FIXED IT! Seems to me I booted with extensions off and the card and holder
stayed in, then I restarted (this time allowing extensions to load) and all
has been fine since...
