Try this:

Take out the USB card, go through complete start up and shut down. Put the
USB card back and reset the PRAM on start up (or by taking out the battery.)

On advise from one of my colleagues at work, this is what I went through to
make my USB card work.

Subject: PC 150 and PCI Cards

I recently moved my USB PCI card to the outermost slot to make room
for my Radeon 7000 card in the center. However, my USB Zip drive is
no longer recognized when I plug it in. It doesn't even make a
whirring sound on startup like it used to. It used to work fine with
the card in the center position. Ideas, anyone? OWC said it should
work in any position. Are there any circuit differences between
slots? Must be.

PC 150 Tower with 604 chip (soon to get a G3 upgrade, that is, if I
can lick this problem),three PCI slots, ATA 100 controller in the
innermost slot, 40GB IDE drive.
