It's the same with automobiles, motorcycles and airplanes.
Suspect the last thing you did, before the malfunction appeared, i.e., the way you wired in the power supply.
I've only had to change two in the last 18 years (Apple IIvx and PowerComputeing Power 100) and neither was a problem.
(With that smug statement, I may have jinxed myself.)
